Your medical record is strictly private and confidential. We will NOT release information regarding your condition, either orally or in writing, to your employer, relatives, or friends without your written permission, except as required by law.

All staff members have been trained and follow the HIPPA guidelines.

Copies of your records require a signed release and a minimum of ten (10) working days to process copies of chart pages. Reproduction of photographic materials may require additional time, over and above the ten (10) working days, and will be billed to you at the cost of production.
